The key to solving a problem is to find the Root Cause of the problem

In the long term it is not beneficial to treat the effect of a problem as this is "Fire Fighting“

If necessary you must put an immediate temporary countermeasure in place to ensure that the customer is not affected by the problem. You must then eliminate the root cause to prevent future re-occurrence

Using ‘Cause - Effect‘ diagrams to aid in problem solving, the team brainstorms root cause problems associated with MAN , MACHINE, MATERIAL and METHODS. These issues are then brainstrormed using 5 WHY technique to establish root cause of the problems and convert into ACTIONS.
